* Photo ID (16 and over)
* Certified Copy of Birth Certificate

* Fee:  Adult 16 Years and Up = $145 (Personal Check or Money Order Made Out To Department Of State for $110 plus Cash/Personal Check/Money Order for $35 to the Crawford County Circuit Clerk)

                   Children  (15 Years and under) = $115 (Personal Check or Money Order Made Out To Department Of State for $80 plus Cash/Personal Check/Money Order for $35 to the Crawford County Circuit Clerk)

Under 16 - Signatures of both parents are required.
